Job Overview

Required Qualifications
- Valid Occupational Therapist license in New Mexico
- BLS - Basic Life Support issued by American Heart Association or American Red Cross
Required Health and Compliance Items
- Background check required, including a 7-year criminal check and review of all locations lived/worked in the past 7 years
- Physical exam within 12 months of start
- Negative TB test or chest X-ray within 1 year of start
- Tdap within the past 10 years
- Measles, mumps, rubella, and varicella immunity documentation required
- Hepatitis B documentation required
- Drug screen required within 90 days of start
- Flu vaccination or declination required during flu season
- Fit testing completed onsite
- Core competency exams required before start
- Orientation and facility policy acknowledgements required before start
Certifications
- BLS required
- ACLS required as applicable
- PALS required as applicable
- NRP required as applicable
- TNCC required as applicable
Additional Notes
- 75-mile radius restriction applies
- Photo ID must be carried on person during hours of service
- COVID declination form is available if declining vaccination
