Physical Therapist - Home Health

Job Overview

Contract role
Full-time, day shift
32 hours per week
Home health setting
Service area includes White River Junction, Lebanon, Bradford, and Windsor

Required Qualifications

Bachelor's degree in Physical Therapy
Valid Physical Therapist license in Vermont
Physical Therapist (1 year of experience)
Physical Therapist experience within the last year
BLS
Experience in a Home Health setting

Key Responsibilities

Evaluate patients, develop treatment plans, and carry out therapy programs
Screen, re-evaluate, and treat patients
Communicate and coordinate care with the interdisciplinary team
Document evaluations, treatment goals, and objectives
Teach patients and family members about treatments and procedures

Work Environment

Average patient visit is 45 minutes
Broad independence with support from a cohesive care team

Marigold is polite and professional. She goes above and beyond to ensure the contractor is well informed and has everything needed to be successful. She helped me with onboarding and getting used to the timesheet app and she answers my texts promptly, even after hours. She is a great recruiter liason. Gladys was a super helpful with onboarding and Denise will hook you up with benefits with no trouble at all. Great team to work with.

Cons

Onboarding is somewhat tedious, but they do dot every "i" and cross every T.
