OT Home Health
Overview

- Schedule note: This facility cannot accommodate 4x10s or 3x12s
- Setting: Home Health
Work Expectations
- 1:1 patient ratio
- May visit up to 6 patients per day
- Standard productivity: 30 points
- No weekend requirement
- No on-call requirement
- No floating between like units
- Block scheduling is not available
- Traveler should have their own transportation
- Occasional daily travel radius may be up to about 60 miles, with 60–90 minutes total drive time
Required Qualifications
- Valid Occupational Therapist license in Oregon
- Occupational Therapist (1 year of experience)
- Occupational Therapist experience within the last year
- BLS issued by American Heart Association or Heart & Stroke Foundation
- Experience in a Home Health setting
- Experience with OASIS
Additional Notes
- First-time travelers are accepted
- Local candidates are accepted
- RTO must be confirmed with the hiring manager
- Guaranteed hours may be reduced by up to one shift per pay period

- Employment application covering at least the last 5 years
- Minimum of 2 references covering 1 year of employment total within the last 3 years in a comparable specialty/setting
- Skills checklist
- Physical exam within 1 year of start
- Drug screen within 12 months of start
- TB clearance
- Measles, mumps, rubella, varicella, hepatitis B, tetanus/diphtheria/Tdap, flu, and COVID documentation or declination as applicable
- Respiratory safety clearance
- Washington border-state screening and disclosure items may apply for licensure to work in Washington
Work Environment
- Business casual attire; scrubs allowed but not required
- Free parking
- Mileage reimbursement at IRS standard rate
- Charting includes EPIC Remote Client for Home Health, Rover, and Oasis
