Job Overview

Required Qualifications
- Valid RN license in Washington
- House Supervisor experience within the last year
- 2 years of House Supervisor experience
- Associate of Science in Nursing / Associate Degree in Nursing
- ACLS - Advanced Cardiac Life Support issued by American Heart Association or American Red Cross
- BLS - Basic Life Support issued by American Heart Association or American Red Cross
Compliance and Onboarding
- Background check required within 30 days of start
- Washington State Patrol/WATCH search required within 30 days of start
- EPLS/GSA/SAM check required within 30 days of start
- HHS/OIG check required within 30 days of start
- Education verification required
- 3 recent employment references required
- Employment application required
- Photo for badge required
- Code of Conduct acknowledgement required
- New Employee Information Sheet required within 48 hours of signing contract
- Student and Contractor Onboarding required
- Statement of Confidentiality required
- Confidentiality and Use Agreement required
- Acknowledgement Badge Form required
Health Requirements
- Physical health statement signed by a physician within 1 year of start
- Fit testing performed onsite
- Tdap vaccine or signed declination required
- Negative TB test or chest X-ray required within 30 days of start
- TB questionnaire required annually
- Measles, mumps, and rubella immunity documentation required
- Hepatitis B immunity documentation required
- Varicella immunity documentation required
- 12-panel drug screen required within 30 days of start
- Flu vaccination or declination required by 11/01
