Role summary
- Evaluates and provides medically prescribed physical therapy treatment.
- Performs initial and ongoing patient assessments.
- Develops, revises, and evaluates plans of care with patient/family involvement.
- Provides therapy interventions using standard physical therapy techniques.
- Oversees physical therapy assistants, students, and other support personnel.
Required qualifications
- Valid Physical Therapist license in Oklahoma
- Physical Therapist (2 years of experience)
- Physical Therapist experience within the last year
- Driver's License

- MMR documentation
- Hepatitis B vaccination documentation or declination
- Varicella documentation
- Tdap vaccination
- TB screening documentation
- Fit test
