Required Qualifications

• Valid Physical Therapist license in Massachusetts
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• Experience in a Long Term Care or Rehabilitation setting

Job Responsibilities

• Evaluate, screen, and rehabilitate patients with physical functioning disorders and/or wound care
• Develop and adjust plans of care based on patient progress and outcomes
• Perform rehabilitation interventions such as manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive devices, and wound care
• Provide consultation and counseling to patients, families, caregivers, and other service providers
• Communicate patient progress, barriers, and treatment plans with the supervisor and care team
• Complete discharge summaries and required documentation in the electronic documentation system
• Record treatment minutes accurately
• Participate in required meetings, trainings, in-services, and conferences
• Obtain necessary DME or supplies
• Supervise and support physical therapist assistants, rehabilitation aides, and/or students

Practice Expectations

• Follow regulatory and clinical practice standards
• Adhere to APTA, state practice acts, code of conduct, and ethics
