PT Outpatient
Job Overview

Required Qualifications
- Valid Physical Therapist license in North Carolina
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- Experience in an Outpatient setting
Compliance / Onboarding
- Government-issued photo ID required
- Background check required within 1 year of start, covering all locations lived/worked/in school in the past 7 years
- EPLS/GSA/SAM screening required within 30 days of start and at each renewal/extension
- HHS/OIG screening required within 30 days of start and at each renewal/extension
- NPI number required and verified before start
- Primary source verification of state license required
- Two clinical references required; one must be from a supervisor or higher
- Complete application/resume required
- Skills checklist required annually
Health Requirements
- Physical exam required within 1 year of start
- Tdap required within 10 years of start
- Negative TB test or chest X-ray required, with annual update
- Measles, mumps, rubella, hepatitis B, and varicella documentation required
- 10-panel drug screen required within 30 days of start
- Flu vaccine or approved medical/religious exemption required during flu season
Additional Notes
- Start date is mid to late September
- Candidates with approved flu exemptions must wear a mask in patient care areas
