Hospice RN Case Manager
Location
- Missoula, Montana

Schedule
- Typical schedule is Monday–Friday, 8:00 AM–5:00 PM
- On-call and weekend rotation
Role Summary
- Provides skilled nursing and case management for patients in their place of residence
- May start as a Visiting RN and transition to Case Manager
- Coordinates care, supports continuity of care, and uses the nursing process in an interdisciplinary team setting
Required Qualifications
- Valid RN license in Montana
- Hospice experience within the last year
- 2 years of hospice experience
- BLS
- Associate of Science in Nursing / Associate Degree in Nursing
- Driver's License
- Appropriate auto insurance
- Reliable transportation
Preferred Experience
- 2 years of recent acute care, home care, or hospice experience
- Experience with venipuncture, catheterization, wound care, and/or parenteral therapy
- HCHB experience
Additional Notes
- BSN preferred
- Maintains patient records in accordance with agency standards and OASIS requirements
