Hospice RN Case Manager

Location

  • Missoula, Montana

Employment Type

  • Contract

Schedule

  • Typical schedule is Monday–Friday, 8:00 AM–5:00 PM
  • On-call and weekend rotation

Role Summary

  • Provides skilled nursing and case management for patients in their place of residence
  • May start as a Visiting RN and transition to Case Manager
  • Coordinates care, supports continuity of care, and uses the nursing process in an interdisciplinary team setting

Required Qualifications

  • Valid RN license in Montana
  • Hospice experience within the last year
  • 2 years of hospice experience
  • BLS
  • Associate of Science in Nursing / Associate Degree in Nursing
  • Driver's License
  • Appropriate auto insurance
  • Reliable transportation

Preferred Experience

  • 2 years of recent acute care, home care, or hospice experience
  • Experience with venipuncture, catheterization, wound care, and/or parenteral therapy
  • HCHB experience

Additional Notes

  • BSN preferred
  • Maintains patient records in accordance with agency standards and OASIS requirements

Marigold is polite and professional. She goes above and beyond to ensure the contractor is well informed and has everything needed to be successful. She helped me with onboarding and getting used to the timesheet app and she answers my texts promptly, even after hours. She is a great recruiter liason. Gladys was a super helpful with onboarding and Denise will hook you up with benefits with no trouble at all. Great team to work with.

Cons

Onboarding is somewhat tedious, but they do dot every "i" and cross every T.
