Outpatient Physical Therapist
Job Overview
- Travel/contract outpatient physical therapist role in Yukon, Oklahoma.
- Treats adult patients with various orthopedic conditions.
- Day shift: 5x8 hours, 8:00 AM to 5:00 PM.
- 8-week contract.
Required Qualifications
- Valid Physical Therapist license in Oklahoma.
- BLS issued by American Heart Association or American Red Cross.
- Driver's License.
Additional Compliance Items
- Hepatitis B documentation required.
- Flu vaccination or approved exemption required by the stated seasonal deadline.
- COVID vaccination is strongly preferred; declinations are acceptable.
- Tetanus/diphtheria/Tdap documentation required.
- Measles, mumps, rubella, and varicella immunity documentation required.
- Hepatitis A documentation required for pediatric/PICU positions and caregivers for the specified campuses.
