Role Overview

• Full-time travel/contract position in home care.
• Coverage area: Dayton, Brookville, Eaton, and West Alexandria.
• Schedule: Monday–Friday, no weekends required; weekend visits are optional.
• Patient population: adults and geriatrics.
• Weekly productivity expectation: 7 visits per day or 35 visits per week.
• Longer contract available through the end of March.

Required Qualifications

• Valid Physical Therapist license in Ohio.
• Physical Therapist (1 year of experience).
• Physical Therapist experience within the last year.
• BLS - Basic Life Support issued by American Heart Association.
• Current resume/application required.
• 2 references from within the last 2 years, with 1 from a manager, supervisor, or charge RN.
• Skills checklist required within 1 year prior to documentation due date and updated annually.
• Physical exam within 2 years of start.
• Negative TB test or chest X-ray.
• Drug screen within 12 months prior to documentation due date.
• Annual health questionnaire.
• Candidate agreement.
• E-Verify.
• Government-issued photo ID.
• Proof of name change, if applicable.
• 7-year criminal background check.
• EPLS/GSA/SAM check.
• HHS/OIG check.
• Respiratory protection medical questionnaire.
• RespSafety clearance within 1 year of start.
• Work history verification if 2 professional references cannot be obtained.

Preferred

• Weekend visits if the clinician chooses.
• Home Care Home Base familiarity; training will be provided if the candidate does not already know the system.
