Home Health RN

Overview

• Travel/Contract role in Portland, Oregon
• Home Health assignment based in East Portland and surrounding areas
• Start date: ASAP
• Shift: 8-hour days, 5x8 schedule
• First-time travelers accepted
• Local candidates accepted within a 50–75 mile radius
• No weekend requirement
• No on-call requirement
• No floating between like units
• Block scheduling not available
• Facility can call off up to one shift per pay period
• Traveler must have their own transportation
• May visit up to 6 patients per day
• Standard productivity: 30 points
• Occasional travel radius of roughly 60 miles per day
• Oasis experience is required
• Primary series COVID vaccination plus one booster is required
• EPIC Remote Client for Home Health and Rover are used for charting

Required Qualifications

• Valid RN license in Oregon
• Home Health experience within the last year
• 1 year of Home Health experience
• BLS issued by American Heart Association or Heart and Stroke Foundation
• Associate of Science in Nursing / Associate Degree in Nursing
• Experience with OASIS
• NURSYS verification required for all RNs
• State license verification required for all RNs
• Background check within 90 days of start covering all locations lived/worked within the past 7 years
• Education verification required
• Employment application with at least the last 5 years of employment
• Minimum of 2 references covering 1 year of employment total within the last 3 years in a comparable specialty/setting
• Washington State Patrol search is required because this is a border facility and the role is licensed to work in Washington
• Washington disclosure statement required for placements with valid licensure to work in Washington
• Oregon–Washington meal and rest period waiver required for candidates floating to a Washington facility
