Overview

• Travel/Contract home health hospice case manager
• Location: Onley, Virginia
• Day shift: 5x8 hours, 8:00–16:30
• 40 hours per week
• Contract length: 13 weeks
• Locals allowed at same rate
• Mileage reimbursed at IRS rate
• No call

Required Qualifications

• Hospice experience in the home health setting
• Valid and current driver’s license
• E-Verify
• 2 professional references
• Application/Resume required
• Current BLS
• Skills Checklist 1
• Annual skills checklist update
• Annual background check
• Annual TB questionnaire
• Drug screen within the last 12 months
• Physical exam within the last 12 months
• Proof of 2 PPDs at least 2 weeks apart within the last 12 months, or a QGold/T-Spot completed within 12 months of start date, plus the TB screening questionnaire showing no symptoms
• Virginia State Police Check cleared prior to start
• MVR required for Home Health and Hospice facilities/specialties
• Minimum 3 years of driving experience
• Minimum age 21, or 25 if transporting patients
