Occupational Therapist Pediatrics
Overview
- Travel/Contract role in Willmar, Minnesota
- 13-week assignment
- Day shift: 4x10 hours, 7:30 AM–6:00 PM
- Mostly pediatric hospital outpatient with some adult inpatient coverage as needed
- Working with both adult and pediatric patients with a wide variety of diagnoses
- Outpatient clinic is attached to the hospital via a tunnel and is across the street
Required Qualifications
- Valid Pediatric Occupational Therapist license in Minnesota
- Pediatric Occupational Therapist (1 year of experience)
- Pediatric Occupational Therapist experience within the last year
Required Credentials and Compliance
- Occupational Therapist license, if applicable
- BLS certification, if applicable
- Physical exam within 1 year of start
- Fit mask required
- Tdap vaccination required
- Negative TB documentation required
- Rubeola (Measles) immunity documentation required
- Mumps immunity documentation required
- Rubella immunity documentation required
- Varicella immunity documentation required
- Hepatitis B vaccination, positive titer, or declination required
- Flu vaccination documentation or declination required during flu season
- Criminal background check required within 1 year of start
- GSA/SAM excluded provider check required
- OIG excluded provider check required
- Third-party education verification required
- Photo for badge required
- Confidentiality agreement required
