Home Health RN
Overview
- Travel/contract role in Lebanon, Pennsylvania
- Home care assignment covering Adams and Franklin Counties
- Day shift: 8:00 AM–4:30 PM, Monday–Friday
- 40 hours per week
- Every 4th weekend rotation
- Contract length: 13 weeks
Required Qualifications
- Valid Pennsylvania RN license
- Home Health experience with 2 years of experience
- Home Health experience within the last year
- Associate of Science in Nursing / Associate Degree in Nursing
- BLS issued by American Heart Association or American Red Cross
- OASIS
- 2 professional references within 3 years of start
- Most recent resume/application
- Skills checklist
- Acknowledgement of Pass/Fail Testing
- State license verification
- NURSYS verification
- Background check, including Pennsylvania PATCH search
- Pennsylvania Child Abuse Search
- Pennsylvania Department of Human Services fingerprint results
- Pennsylvania Department of Aging fingerprints for non-PA residents
- Education verification
- Physical exam within 90 days of start
- Fit mask testing within 1 year of start
- Tetanus/Diphtheria/Tdap within 10 years of start
- Negative TB or chest X-ray within 90 days of start
- Rubeola, Mumps, Rubella, and Varicella immunity documentation
- Hepatitis B declination
- Drug testing
- COVID-19 vaccine, exemption, or declination
- Flu vaccine documentation or valid exemption during flu season

- Travelers are expected to work 2 of 3 major holidays; if the contract overlaps only 2 of those holidays, 1 holiday must be worked

- RTO maximum: 7 days
