Assignment Overview

• Travel/Contract role in Hays, Kansas
• 13-week contract
• Day shift: 4x10 hours, 7:00–17:00
• 40 hours per week
• Weekday and weekend call rotating with current heart team

Required Qualifications

• CVOR Technologist with 1 year of experience
• CVOR Technologist experience within the last year
• BLS issued by American Heart Association
• NBSTSA
• Government issued photo ID required and must be carried during hours of service
• Social Security Card or SSN required
• Proof of E-Verify or attestation required
• Background check required, including 7-year criminal check, SS trace, counties nationwide, sex offender, FACIS Level 3, and education verification
• EPLS/GSA/SAM required within 30 days of start and at each renewal/extension
• HHS/OIG required within 30 days of start and at each renewal/extension
• State license verification required
• Medication test/pharmacology competency exam required as applicable by profession
• Competency exam required as applicable to profession/specialty
• Physical exam within the last 12 months
• Tdap vaccination within the last 10 years
• Negative TB or chest X-ray documentation required
• Rubeola, mumps, rubella, hepatitis B, and varicella immunization documentation required
• Negative 13-panel drug test required within 30 days of start
• Vaccination declination, TBQ form, and respirator clearance required prior to start
• Flu vaccination or exemption required seasonally
• References required for travel positions
• Skills checklist required for applicable unit
• Employment application/resume required

Preferred

• Tech cert verification preferred for CVOR Tech
• Orientation safety manual acknowledgement preferred for subs
• Core Part I, Core Part II, and Core Part III may be completed by subs instead of signed Lotus HIPAA form
