Role Summary
- RN case manager providing skilled care and case management to patients and families in the home health setting.
Required Qualifications
- Valid RN license in Massachusetts.
- Home Health experience within the last year.
- 1 year of Home Health experience.
- Associate of Science in Nursing / Associate Degree in Nursing.
- BLS issued by American Heart Association or American Red Cross.
- Driver's License.
- Resume required for submission.
- 2 references required for submission, with at least 1 supervisor or above in the skillset.
- Skills checklist required for submission.
- MGB Massachusetts CORI required for onboarding.
- MGB Criminal Background Check required for onboarding.
- MGB SSN Trace required for onboarding.
- MGB National Sex Offender Check required for onboarding.
