RN Case Manager

Overview

This is primarily a triage position. The Registered Nurse provides professional nursing care across the lifespan, using the nursing process to assess, plan, implement, and evaluate patient care. The RN collaborates with providers and interdisciplinary teams to support patient safety and outcomes.

Responsibilities

• Perform comprehensive assessments, develop individualized plans of care, and administer medications and treatments per provider orders and policy.
• Recognize changes in patient condition and initiate interventions or escalation of care.
• Provide patient and family education on care, medications, and discharge needs.
• Coordinate care with providers and team members, including referrals, follow-up, and continuity of services.
• Document assessments, interventions, and outcomes accurately and on time.
• Report adverse events, near misses, and unsafe conditions per policy.
• Provide guidance and delegation to ancillary staff, and support orientation and mentoring of staff and students.
• Float to other departments or assignments as needed.

Required Qualifications

• Current, unrestricted Registered Nurse license in the State of Alaska, required at time of submission.
• Associate of Science in Nursing (Associate Degree in Nursing) from an accredited program.
• 1 year of case management (CM) experience, completed within the last year.
• Basic Life Support (BLS) certification.

Pre-Start Requirements

• Submit 2 fingerprint cards upon acceptance.
• Start date may change based on fingerprint submission and receipt of State of Alaska background check results.
