Description
Job Description
As a home health hospice nurse (RN HHH), you will deliver palliative care in patients' residences, helping them find comfort. Your patients may be elderly, critically ill, or disabled and will need end-of-life, compassionate care. You will assess patients' conditions and determine individualized care plans. This will include charting observations, administering medication, dressing wounds, educating patients and their families about care, and coordinating with other medical professionals involved in patients' care plans.
Minimum Requirements
- At least 1 year of recent acute care experience in specialty
- Active RN license
