Description

Job Summary: Hospice Registered Nurse Case Manager (RN) Basking Ridge, NJ. The hospice RN Case Manager is responsible for conducting patient assessments to determine hospice appropriateness, developing and revising the plan of care, and supporting progress toward goals for patients and families. The role also ensures appropriate utilization of services, supports case management activities, and maintains documentation requirements in alignment with hospice regulations and interdisciplinary collaboration. Candidates are required to bring their license on the first day.

Job Details:

  • Schedule: Days (5 days per week), 8a4p; 37.5 minimum hours per week. On-call or weekends required: Yes (1 weekend per month).
  • Required Qualifications: Current New Jersey RN License; Basic Life Support (BLS); Associates degree (ASN) or Diploma with RN License in New Jersey; valid New Jersey drivers license; experience using an Electronic Medical Record (EMR); excellent critical thinking and communication skills; 23 years as an RN in a home hospice environment.
  • Preferred Qualifications: 3+ years as an RN experience.
  • Assesses patients for hospice care appropriateness, learning needs, and progress toward established goals.
  • Develops the plan of care for the patient and family and revises the plan as needed.
  • Ensures appropriate utilization of services while maintaining visit productivity standards.
  • Completes admission assessments and transfers within 24 hours, and completes discharge assessments, including laptop data transfer twice daily.
  • Completes revisit documentation and transfers.
  • Uses age-appropriate teaching methods for patients and families; evaluates and revises the teaching plan as necessary; documents patient education tools used.
  • Assumes responsibility for case management by prioritizing care needs and developing/implementing/evaluating an evidence-based plan of care.
  • Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines and maintains evidence of interdisciplinary communication.
  • Performs Home Health Aide (HHA) supervisions per standards and follows the scheduling process.
  • Documents case management activities and communicates progress toward goals with the interdisciplinary team (IDT).
  • Attends and participates in IDT meetings every 2 weeks and evaluates the plan of care and revises as needed.
  • Maintains a safe environment for patients and coworkers by following infection control measures and reporting infections.
  • Demonstrates knowledge of pharmacology and safe medication management, including routine medication reconciliation and maintaining current medication orders.
  • Participates in monthly staff meetings and performance improvement activities as requested, including reviewing health records and gathering data to support performance improvement (PI) initiatives.
  • Attends utilization review (UR) meetings as requested.
  • Provides orientation and mentorship to new employees and cross-trains to learn new skills and support knowledge sharing.
  • Conducts monthly staff meeting participation, participates in PI meetings and activities/committees as requested, and accepts other assignments as needed.
  • Float: Float: Float rotation may be required.
