Description

Job Summary:
The location is in New York, NY. This nursing supervisory role involves assisting with the planning, organizing, directing, coordinating, and evaluating of nursing services within the transitional care division of care management. The position supports efforts to reduce 30-day readmissions and serve high-risk patient populations discharged from inpatient medical-surgical units or emergency departments. Candidates are required to bring their nursing license on the first day.

Job Details:

  • Schedule: Days shift, 7.5 hours per day, with a minimum of 37.5 hours per week.
  • Required Qualifications: Bachelor's degree in Nursing from an accredited nursing program; RN License; Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED); BLS certification; 7 years of experience, including 5 in a supervisory role; at least 2 years of experience in discharge planning and utilization review; 2 years of experience in care management supervision; skills in discharge planning, utilization review, care management, supervisory experience, patient care improvement, program development, performance monitoring, and record-keeping system development.
  • Preferred Qualifications: Master's degree in Nursing.
  • Additional Information: The role involves providing oversight to the care management team, supervising care coordinators and account managers, and supporting program development and performance monitoring related to discharge and care coordination.
