Job Summary
RN (Registered Nurse), Clinic Traveler Pagosa Springs, Colorado. The organization is seeking two experienced registered nurses to support outpatient care within a rural health primary care clinic. This role partners with the clinical care team to assist with patient triage, care coordination, patient education, medication management, and chronic disease management while contributing to a positive patient experience. Candidates are required to bring their license on the first day.
Job Details
- Schedule: MondayFriday, 10-hour day shifts (40 hours/week).
- Required Qualifications: Current Colorado RN license or active compact RN license in good standing; Basic Life Support (BLS) certification; Minimum two (2) years of recent clinical nursing experience; Cerner electronic medical record (EMR) experience required (must be listed on resume); Strong assessment, triage, patient education, and care coordination skills; Ability to work independently in a fast-paced outpatient clinic environment.
- Preferred Qualifications: Previous rural health clinic, family practice, internal medicine, or primary care experience; Experience with immunizations, medication administration, and nursing triage; Clinical laboratory testing experience involving CLIA-waived procedures; Experience supporting quality improvement initiatives and patient engagement programs.
- Responsibilities: Provide comprehensive nursing care in an outpatient clinic setting; Perform patient assessments, triage, care planning, treatment implementation, and follow-up; Manage patient phone calls, portal messages, and nursing advice using established protocols; Administer medications, injections, immunizations, and other clinic-based treatments; Perform point-of-care testing and CLIA-waived laboratory procedures; Coordinate care with providers, specialists, and ancillary departments; Complete accurate and timely documentation in the electronic health record; Support quality improvement initiatives and patient engagement efforts; Serve as a clinical resource for patients and clinic staff.
