Description

Job Summary: Wound Care Coordinator (Dialysis Unit), Greensboro, North Carolina. The organization is seeking a Wound Care Coordinator to manage the facility wound care program, including standards of care and practice related to wound, ostomy, and continence patient needs. The role includes providing direct patient care, supporting and advising staff nurses, developing and implementing wound care protocols and documentation tools, and coordinating ongoing staff education. Candidates are required to bring their license on the first day.

Job Details:

  • Schedule: Days shift, 8:00 AM4:30 PM (8 hour shifts), 5 days/week, for a minimum of 40 hours per week; start date: 07/20/2026.
  • Required Qualifications: Basic Life Support (BLS) certification (American Heart Association required, and must be attached to the submission); North Carolina Registered Nurse (RN) license or multi-state RN license (must be attached to the resume); 2 years of recent Wound Care Registered Nurse (RN) or Wound Care Coordinator experience in a hospital setting.

  • Additional Information:
    • Manages the facility wound care program by developing and implementing patient care protocols, documentation tools, wound care formulary, and Wound Care Clinician referral criteria.
    • Establishes standards of care, competencies, policies, and procedures for clinical staff to support quality, cost-effective, and effective wound care.
    • Serves as a primary consultant to Wound Care Clinicians and provides training and support to enable clinical staff to assess wounds, recommend protocols, and initiate plans of care.
    • Provides ongoing education to staff on products available for use in the hospital.
    • Evaluates all wounds upon admission and ongoing to determine treatment plans and support early problem identification; provides consultation and/or assessment for pressure injuries.
    • Consults on wounds without measurable signs of healing within two weeks; collaborates with the physician to assist with comprehensive wound management and prevention plans of care.
    • Recommends changes to wound care orders and provides evidence-based research support as needed.
    • Ensures wound assessments, care plans, and treatments are documented accurately and that appropriate wound-related treatments are provided.
    • Investigates wound-related adverse events through completion of root cause analysis (RCA) and develops action plans with nursing leadership.
    • Participates in clinical outcome monitoring, follow-up, and agency performance improvement initiatives.
    • Supports CMS quality data reporting by completing wound-related LTRAX data set records.
    • Collaborates with Support Center Clinical Operations staff to maintain a cost-effective wound care formulary.
    • Assists with training of new hire Wound Care Coordinators within the hospital division.
