Hospice Registered Nurse Case Manager (RN) Basking Ridge, New Jersey

The healthcare provider is seeking a Hospice Registered Nurse Case Manager to support patients and families by assessing eligibility for hospice care, developing and updating care plans, and ensuring appropriate utilization of services. Responsibilities include coordinating case management activities, maintaining accurate clinical documentation, supporting interdisciplinary communication, and following hospice regulations and infection control measures. Candidates are required to bring their license on the first day.

Job Details

• Schedule: Days, 7.5 shift hours; 5 days per week; minimum 37.5 hours per week. On-call or weekends required: 1 weekend per month.
• Required Qualifications: Current New Jersey Registered Nurse (RN) license; Associate of Science in Nursing (ASN) degree OR diploma with RN licensure in New Jersey; Basic Life Support (BLS); valid New Jersey drivers license; 23 years as RN in a home hospice environment; experience using an electronic medical record (EMR); excellent critical thinking and communication skills.
• Preferred Qualifications: Three (3) or more years of RN experience.
• Responsibilities: Performs patient assessments to determine appropriateness for hospice care, evaluates learning needs, and monitors progress toward goals; develops the plan of care for the patient and family; ensures appropriate utilization of services and maintains visit productivity standards; completes admission, revisit, and discharge assessments/documentation, including transferring required data as specified in the workflow; uses age-appropriate teaching methods and documents patient education tools used; assumes responsibility for case management by developing, implementing, and evaluating the plan of care based on evidence-based knowledge, prioritizing care needs, and revising care plans as indicated; demonstrates knowledge of hospice regulations and insurance reimbursement guidelines and provides evidence of interdisciplinary communication; attends and participates in interdisciplinary team (IDT) meetings every 2 weeks; participates in monthly staff meetings and PI (Performance Improvement) meetings/activities as requested; performs Home Health Aide (HHA) supervisions per standards; maintains a safe environment for patients and coworkers by following infection control measures and reporting infections; demonstrates knowledge of pharmacology and safe medication management, including routine medication reconciliation and maintaining current medication orders; provides orientation/mentorship to new employees, cross-trains to learn new skills, and assists with growing agency business as requested; accepts other assignments as needed.
• Float: Float: Float rotation may be required.
