Description

Job Summary: Physical Therapist (Home Healthcare) Missoula, Montana
The Physical Therapist provides evaluation and rehabilitation interventions in the home setting. The role includes developing and managing individualized plans of care, delivering direct patient treatment, and educating patients and families. Candidates are required to bring their license on the first day.

Job Details:

  • Schedule: Nights; minimum 36 hours per week, up to 12 shift hours, typically Monday through Friday, with rotating on-call, weekend, and holiday coverage.
  • Required Qualifications: BS degree or better from an accredited Physical Therapy program; current license to practice in the State of Montana; valid drivers license; appropriate auto insurance.
  • Preferred Qualifications: Two years of previous Physical Therapy experience.
  • Responsibilities:
    • Evaluates and establishes a plan of care to meet patient needs in the home environment, including coordinating care with other disciplines and seeking orders for appropriate additional services.
    • Provides direct patient care according to the established care plan and physician orders.
    • Re-evaluates patient status and progress, modifying treatment as appropriate (including frequency and duration of care).
    • Communicates appropriately with physicians and other staff members.
    • Documents all skilled services provided to patients.
    • Communicates with the scheduler each day worked to support consistent scheduling.
    • Oversees services provided by a Physical Therapist Assistant (if applicable).
    • Performs other duties as assigned by the supervisor.
