Job Summary: Telemetry (Telemetry), Lebanon, New Hampshire Skilled Nursing, Sub-Acute, and Dementia Unitsprovides nursing care for long-term residents, including medication administration and monitoring of resident health status. The role supports nursing care planning through assessment and ongoing observation, while collaborating effectively to respond to changes in condition. Candidates are required to bring their license on the first day.

Job Details:

• Schedule: Nights; 8 hours per shift; 40 hours per week (8, 12, and 16 hour shifts are optional).
• Required Qualifications: Valid Compact License; experience and/or proficiency working in Skilled Nursing, Sub-Acute, and Dementia Units; proficiency with memory care techniques and protocols; knowledge of Minimum Data Set (MDS) assessments for long-term care facilities; familiarity with case management practices; ability to work effectively in a medical office environment; excellent interpersonal skills, communication, and task management strategies; ability to assess resident health problems and develop and implement nursing care plans; ability to monitor resident progress and respond to changes in condition; familiarity with Emergency Services (SUD) and pain management support; proficiency with Electronic Medical Record (EMR) systems, Point Click Care (PCC).
• Preferred Qualifications: Familiarity with some intravenous (IV) skills, IV medications, tube feedings, and light wound care.
