Job Summary:
Telemetry Nursing Specialist in Lebanon, New Hampshire. This position involves providing care to long-term residents with a focus on telemetry monitoring, medication administration, and resident assessment. The role requires proficiency in memory care techniques, familiarity with MDS assessments, and the ability to develop and implement nursing care plans within a skilled nursing environment. The assignment is scheduled to start as soon as possible and will follow night shift hours.

Job Details:

• Location: Lebanon, New Hampshire
• Shift Schedule: Night shifts, 8 hours each, with flexible options for 12 and 16-hour shifts.
• Minimum Hours Per Week: 40 hours
• Required Qualifications: One (1) year of experience in telemetry or related nursing specialty; valid nursing license with a compact license; ability to work night shifts; proficiency in EMR: PCC (Point Click Care); strong case management background; excellent interpersonal, communication, and task management skills; knowledge of long-term care, sub-acute, and dementia unit operations; familiarity with IV skills, IV medications, tube feedings, and light wound care preferred.
• Preferred Qualifications: Familiarity with memory care protocols, MDS assessments, and care support involving PT, OT, SUD, and pain management.
• Additional Information: Care involves managing residents health problems, monitoring progress, and responding to changes in condition to ensure quality resident care.
