Job Summary
Home Dialysis Registered Nurse (RN) (Dialysis Unit) Cooperstown, New York
The organization is seeking a Home Dialysis Registered Nurse (RN) to support home-based dialysis care across multiple clinic locations within the Cooperstown, Oneonta, and Little Falls area. The role includes ongoing patient assessments and clinical management, modality education and training, troubleshooting and triage of patient concerns, and coordination of care to support patient independence in the home setting. Candidates are required to bring their license on the first day.
Job Details
- Schedule: Rotating schedule; 4 x 10-hour shifts, 6:00 AM4:30 PM; minimum 36 hours per week.
- On-Call Requirements: Weekday call coverage is typically 4:30 PM6:00 AM, and weekend call coverage includes 24-hour coverage Saturday and Sunday. Call rotation is typically every other week.
- Responsibilities:
- Manage outpatient home dialysis clinics
- Conduct patient assessments and ongoing clinical management
- Provide modality education and training for new home dialysis patients
- Troubleshoot and triage patient concerns and treatment-related issues
- Coordinate care and support patient independence in the home setting
- Occasionally assist inpatient nursing units with CAPD exchanges
- Complete initial home visits for new patients as needed
- Required Qualifications: Registered Nurse (RN) licensure; ability to participate in a rotating on-call schedule; strong assessment, triage, and patient education skills.
- Preferred Qualifications: Previous home dialysis experience is strongly preferred; Peritoneal Dialysis (PD) experience is highly desirable; experience educating and training dialysis patients is preferred.
- Additional Information:
- Patient population includes approximately 14 home dialysis patients across the network, with Peritoneal Dialysis (PD) as the primary modality. The program supports both Peritoneal Dialysis (PD) and home hemodialysis (HHD).
- Travel between clinic locations in Cooperstown, Oneonta, and Little Falls may be required.
- RNs are only expected to travel to patient homes for the initial home assessment/visit; ongoing patient care is primarily provided in the clinic setting.
- A personal vehicle is required.
- Agency nurses typically shadow an experienced team member during their first week; after orientation, nurses are expected to independently manage their assigned patient population and responsibilities.
