Job Summary
Community Health Nurse (CHN) Home Healthcare, Pittsfield, Massachusetts. The community health nurse provides, teaches, supervises, and documents nursing care for individuals who are sick or injured at home, supporting efforts to promote, maintain, and restore health. The role includes collaborating with healthcare professionals and community representatives to coordinate care, evaluating responses to interventions, and exercising independent clinical judgment under general guidance of the director. Candidates are required to bring their license on the first day.

Job Details

• Schedule: Days, 8 hours per shift, minimum 40 hours per week.
• Required Qualifications: Active Massachusetts Registered Nurse (RN) license (must be uploaded with the profile); Active American Heart Association Basic Life Support (AHA BLS); One year acute nursing care experience (hospital-based acute nursing care preferred); Experience in home health (required); Certified home health credential required (including proficiency with OASIS); BSN highly preferred; Current driver license (and insurance) required; Ability to function independently under general guidance; Demonstrated critical thinking and interpersonal skills; Ability to supervise others; Ability to maintain confidential information; Ability to develop and maintain records and communicate clinical data in verbal and written form; Ability to be flexible regarding work hours; Ability to drive an automobile in inclement weather; Willingness to accept on-call responsibilities; Ability to exercise independent judgment.
• Preferred Qualifications: Infusion therapy certification; Demonstrated knowledge of community health principles and practice; Experience with self-scheduling of caseload; History of flexibility in assuming varied assignments; BSN highly preferred; Hospital-based acute nursing care preferred.
• Additional Information: Traveler candidate must have their own vehicle. On-call responsibilities may be required. Requested time off during the course of the assignment must be noted and verified at upload; after upload/during interview requests will not be approved. No more than 5 days of requested time off will be accepted (less is better). At least two specific interview date/time availability options must be added to the profile (no anytime availability accepted). All scheduling requests must be made at the time of submission; scheduling requests after offer or at/after interview will not be approved.
