Job Summary:
Hospice Case Manager located in Newcastle, Maine. This role involves providing comprehensive hospice care to adult patients, managing a caseload of 14-18 patients based on acuity. Candidates must have a minimum of one year of experience in home health or hospice care. The position requires that candidates bring their license on the first day.

• Required Qualifications: BLS certification, at least 1 year home health or hospice experience, valid license to practice.
• Schedule: Day shift, 40 hours per week, with every other weekend work, on-call rotation overnight from 5 PM to 8 AM on weekdays, and Friday at 5 PM through Monday at 8 AM on weekends.
• Additional Information: On-call requirements are rotated among staff nurses, with an average of one weekday and one weekend per month. No ventilators or invasive monitoring required. Use of in-home infusion pumps is involved, but no ventilators or invasive monitoring. Clinicians will be provided with an agency laptop, iPhone with chargers, vital signs equipment, nursing supply bin, and bags, all to be returned upon departure. Successful self-scheduling and case managing of patients are essential.
• Patient Ratios: Managing 14-18 patients based on acuity, with an expectation of 5-6 visits or units daily, and a minimum productivity of 25 units weekly.
• Experience and Skills: Epic electronic medical record system, PICC line management, dressing changes, trachs, port access, ostomies, chest tubes, Pleurex drains, suprapubic catheters, lab draws/UA/C&S.
