Job Summary:
The location is in Green Bay, Wisconsin. This Home Health Registered Nurse (RN) position involves providing comprehensive patient care within a home health setting. The candidate must have a current Wisconsin RN license or a Compact license and bring their license on the first day. The role requires the ability to work flexible daily shifts, including 8-hour or 10-hour days, with the possibility of extending to 12 hours. Candidates should possess a minimum of 2-3+ years of acute home health experience and be comfortable with hands-on clinical skills, advanced documentation, and remote patient monitoring technology.
Job Details:
- Location: Green Bay, WI
- Schedule: 8-hour or 10-hour shifts (schedule to be discussed); guaranteed 40 hours per week
- Required Qualifications: Current Wisconsin RN license or Compact license; Associates Degree or Certificate from an accredited school of nursing (BSN preferred); 2-3+ years of acute home health experience; EPIC experience required; Point of care documentation experience required; AHA Basic Life Support (BLS); ability to lift up to 50 pounds; valid drivers license and own vehicle for travel between patient sites
- Preferred Qualifications: Experience with wound care, wound vac, ostomies, and hands-on skills; comfort with all home types; experience with electronic monitoring and remote patient monitoring technology; flexibility for on-call and holiday rotations, including every other weekend and holiday
- Additional Information: The role involves travel within a 9-county radius in northeast Wisconsin, with an average of 75-100 miles traveled daily and 4-6 patient visits per day. Staff must be mindful of weather conditions, and safety considerations are prioritized. Provided to team members are a laptop, corporate phone, and clinical bag with vital equipment/PPE. The position includes advanced remote patient monitoring capabilities, including remote auscultation, electrocardiogram capture, and video visits.
