Job Summary
The Medical Surgical Skilled Nursing (MS SNF) Professionally Licensed Registered Nurse (RN) Team Lead position is located in Montour Falls, New York. This role involves overseeing patient care activities within a skilled nursing facility, including assessment, monitoring, and reporting patient conditions. The RN Team Lead collaborates with interdisciplinary teams, promotes evidence-based practices, and maintains clinical competence specific to their care area. The position requires flexibility to act as an LPN Med Cart when staffing demands, and candidates must be available to work every other weekend and holiday.

Job Details

• Schedule: Evenings shift, 8 hours per shift, with a minimum of 40 hours per week.
• Required Qualifications: Valid license as a registered nurse; no minimum experience required; location in or ability to work in Montour Falls, NY.
• Preferred Qualifications: Experience in medical-surgical or skilled nursing facilities is advantageous but not required.
• Float: Float: Float rotation may be required.
• Additional Information: The RN Team Lead is responsible for time management, critical thinking, delegation, and prioritization of patient care. Responsibilities include assessment, monitoring, recording, and reporting changes in patient conditions, advocating for patients, and practicing ethically within their scope. The role involves coordination with interdisciplinary teams, effective communication, and thorough documentation in the electronic medical record (EMR). Physical requirements may include lifting up to 50 lbs., sitting, or standing for extended periods.
