Job Summary:
Home Healthcare Nurse in South Dennis, Massachusetts. This position involves performing comprehensive nursing assessments upon patient admission, developing and revising care plans, and coordinating discharge processes. The role requires ensuring clinical and quality standards are maintained while providing skilled direct care and documentation consistent with regulatory and organizational guidelines.

Job Details:

• Shift Hours: Days, with a schedule of 10 hours per shift, totaling at least 40 hours per week.
• Required Qualifications: Current Registered Nurse (RN) license in Massachusetts, with a minimum of one (1) year of nursing experience or six (6) months of nursing experience with a BSN. CPR certification is also required.
• Certifications: CPR certification.
• Experience: Minimum one year; Home care and IV experience preferred.
• Schedule: Days, 10 hours per shift, minimum of 40 hours per week.
• Additional Information: Performs a comprehensive nursing assessment upon admission, utilizes health resource group values for care planning, documents with clarity, communicates significant patient changes to physicians and care team, and participates in interdisciplinary case conferences. Provides supervision of the Home Health Aide plan of care every 14 days, manages discharges, and ensures timely completion of paperwork.
