Job Summary
The hospice nursing professional with experience in case management (CM EXP) is based in Grand Junction, Colorado. This role involves providing compassionate end-of-life care to patients in their homes or facilities. The primary focus is on symptom management, care planning, and supporting both patients and their families through complex emotional and clinical needs.
Job Details
- Schedule: Days shift, Monday through Friday, 8:00 am to 5:00 pm, with occasional backup for weekends and on-call, minimum of 40 hours per week.
- Required Qualifications: Certified in case management with at least 1 year of relevant experience; valid nursing license; located within the designated geographic area; and ability to work a daily schedule as specified.
- Preferred Qualifications: Experience in hospice or end-of-life care, strong assessment and critical thinking skills, and capability to lead interdisciplinary teams.
- Additional Information: Hospice nurses serve as educational resources for patients and caregivers, assisting them in identifying personal goals of care, managing symptoms, and ensuring quality of life in various care settings.
