Job Summary:
Home Healthcare Registered Nurse (RN) in Portland, Oregon. This position involves providing skilled nursing care to adult and geriatric patients in a home health setting. The role requires delivering comprehensive patient assessments, medication management, wound care, and patient education, ensuring quality care in a community-based environment.

Job Details:

• Schedule: 8-hour days shift, minimum of 40 hours per week.
• Required Qualifications: Oregon State Licensure, BLS certification, 1 year of skilled home health experience, reliable transportation, and COVID-19 vaccine (primary series and one booster).
• Preferred Qualifications: Experience with Oasis, knowledge of disease management, PICC lines, catheter care, and special procedures related to home health.
• Additional Information: The traveler will visit up to six patients daily, with patient ratios managed to ensure quality care. Special attention to infection control precautions, wound care, G-tube site management, and use of charting systems including Epic and Rover is required. The assignment involves minimal drive time within a 60-mile radius, with some travel to multiple locations within Portland. The role involves working in a community setting with interdisciplinary support and interpreting services.
• Submittal Requirements: Provide three available interview times, confirm travelers phone number, list any time-off requests, specify if the traveler has previously worked with the organization, and submit two references covering the past year.
