Job Summary:
Home Healthcare Nurse in St. Albans, Vermont. This position involves delivering comprehensive home health care services to patients, utilizing the nursing process to assess, plan, implement, and evaluate patient care. The role requires collaboration with multidisciplinary teams, documentation accuracy, and patient education to ensure safe and effective care.

Job Details:

• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
• Required Qualifications: Certifications in nursing, Vermont State Board of Nursing licensure, minimum of 0 years of experience, and the ability to work in the United States.
• Responsibilities:
  • Effectively completes OASIS documentation in accordance with agency policies and state and federal regulations, demonstrating knowledge of and adaptability to updates in documentation requirements.
  • Utilizes the nursing process to assess patient and caregiver needs, developing and communicating realistic care plans.
  • Serves as Case Manager for assigned patients, actively managing caseloads and visit times.
  • Implements medical plans of care, nursing interventions, and procedures ensuring safe, effective, and quality patient care.
  • Maintains up-to-date high-tech skills such as IV use, wound VAC, and complex wound care, ensuring adherence to infection control standards.
  • Collaborates with Licensed Practical Nurses (LPNs) to follow care plans, delegate tasks, and meet patient needs.
  • Evaluates the effectiveness of implemented care plans, monitoring patient responses and adjusting as necessary.
  • Ensures accurate and complete documentation of nursing care for continual quality improvement.
  • Performs patient and caregiver education on medication side effects, treatment options, discharge instructions, and reportable conditions.
  • Identifies interdisciplinary needs and communicates relevant information to other healthcare professionals timely.
  • Accurately completes all required agency, state, and federal documentation forms in a timely manner.
  • Manages visits efficiently, coordinating with the scheduler to optimize visit schedules and communicate any scheduling changes.
  • Assesses patient needs for Home Care Assistance (HCA) services, develops, and coordinates plans of care, providing supervision.
  • Transmits patient incident reports promptly and coordinates safe patient discharges or transfers.
  • Collaborates with physicians, providers, team members, and vendors to support patient care plans.
  • Participates in monthly peer review audits and case conferences, providing and receiving feedback professionally.
  • Advocates proactively for patient care needs to ensure high-quality, patient-centered care.
