Description

Job Summary:
Outpatient Registered Nurse (RN) Case Manager position is located in Napa, California. This role involves providing chronic disease case management for a licensed Adult Day Health program serving the frail elderly, younger disabled individuals, and people with Alzheimer's disease and related dementia. The RN will monitor medical conditions in conjunction with the interdisciplinary team and primary care provider within the community setting, offering caregiver education and support. The goal of the program is to reduce emergency department visits and hospitalizations, while preventing premature nursing home placement.

Job Details:

  • Location: Napa, California
  • Shift Schedule: Days, 8 hours per day, minimum 40 hours per week
  • Required Qualifications: Valid California RN license, BLS certification, 1 year of prior RN experience (clinic, primary care, pre/post, or home health settings preferred)
  • Preferred Qualifications: Experience with chronic disease management, family caregiver education, and community health settings
  • Schedule: 8-hour day shifts, starting from 08/10/2026
  • Additional Information: Candidate must have reliable transportation without additional scheduling restrictions; travel pairs are required to have their own transportation. The EMR system used is Turbo TAR, specific to the California Adult Day Health industry. The daily patient ratio is 1:40, with an average census of 72 patients. The role involves providing care for frail elderly and individuals with chronic conditions such as diabetes, hypertension, CHF, COPD, and neurodegenerative diseases.
