Job Summary:
Home Healthcare Nurse in Red Bluff, California. This position involves providing care to patientsmost of whom have recently been discharged from the hospital and are at a high risk for rehospitalization. Responsibilities include patient education and managing conditions such as congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), wounds, wound vacs, IV-PICC lines, and equipment like vents, Cpap, and BiPap. The role requires adherence to industry standards and delivering quality patient care in a home health setting.

Job Details:

• Certifications: BLS (Basic Life Support), valid driver's license, and insurance.
• Education Requirements: Not specified.
• Years of Experience: Minimum of 2 years required.
• Location Requirements: California resident or local candidates preferred.
• Schedule: 8-hour day shifts, five days a week, with flexibility to accommodate weekend and holiday shifts, including Thanksgiving and Christmas. The schedule is typically completed the previous day, with possible morning changes due to start of care processing or sick calls.
• Responsibilities include patient education, utilization review, and managing typical home health equipment such as vents, Cpap, and BiPAP, with an average of about 5 visits per day depending on the schedule.
• Experience or 'Must have' skills: HomeCare HomeBase (HCHB) preferred for documentation and record management.
• Supervisors and support: Manager available during working hours for routine questions; administrator on call for urgent needs during after-hours and weekends.
• Additional notes: The role is in a home health setting, with only home health care aid support available within the department.
• Licensing: Must have a valid license to submit application.
