Job Summary:
Long Term Care Nurse Practitioner position in Beaver Dam, Wisconsin. This role involves providing comprehensive nursing care within a nursing home attached to a hospital, focusing on elderly, dementia, frail, and rehabilitation patients. The position offers night shifts with a focus on clinical assessments, charting, medication management, and specialized skills such as IV therapy and cath care.

Job Details:

• Schedule: 12-hour night shifts with every other weekend and holiday coverage; minimum of 36 hours per week.
• Required Qualifications: BLS certification (AHA); licensure to practice as a nurse practitioner; no minimum years of experience required; location within 80 miles of the facility.
• Preferred Qualifications: Elderly care experience; experience with elderly patient populations.
• Float: Float rotation may be required.
• Additional Information: Responsible for charting, assessments, charge duties, medication administration, and clinical skills including IVs, catheters, and ostomy care. Night shift includes a multidisciplinary team, on-site RT and pharmacy, and requires professional dress in solid, clean attire. Clinicians must have worked for the organization at least six months prior to submission and clock in/out at the facility.
