Job Details:

• Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: CA Registered Nurse (RN) license, 1 year of recent experience within the last 3 years, valid BLS certification.
• Schedule: Days shift, from 8:30 AM to 5:00 PM, with a minimum of 40 hours per week.
• Float: Float rotation may be required.
• Additional Information: Setting involves in-office processing of referrals for home health and hospice. Candidate must be proficient in Health Connect (Epic) charting system and capable of handling referral workflows efficiently.
