Job Summary:
Home Health and Hospice Nurse in Martinez, California. This in-office role involves processing referrals for home health and hospice services within a designated coverage area. The position requires collaboration with healthcare providers to ensure timely and accurate referral management.

Job Details:

• Schedule: Days, 8 hours per day, with a minimum of 40 hours per week
• Required Qualifications: California RN license, BLS certification, at least 1 year of recent experience in the last 3 years, working with Health Connect (Epic) charting system
• Preferred Qualifications: Experience in home health or hospice settings is advantageous
• Additional Information: Setting involves in-office processing of referrals for home health and hospice; coverage area primarily in Northern California. Work hours are from 8:30 am to 5:00 pm, with every other weekend coverage.
