Job Summary:
The organization is seeking a qualified Registered Nurse (RN) for a home healthcare position in Sonora, California. This role involves planning, organizing, and directing in-home care services to meet patient needs. The RN will manage a patient caseload, provide independent patient care, collaborate with other care team members, and serve as a resource for patients and their families in accordance with the care plan.

Job Details:

• Schedule: Days shift, 12 hours per shift, with a minimum of 36 hours per week.
• Required Qualifications: Bachelors Degree in Nursing (BSN) preferred; Acute care facility or home care experience preferred; valid RN licensure in the state of practice; Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification; valid Drivers License and at least 21 years of age; current automobile insurance and reliable vehicle.
• Essential Functions: Conducts initial and ongoing comprehensive patient assessments, develops and adjusts care plans, coordinates with interdisciplinary teams, documents patient care accurately, educates patients and families, and ensures compliance with all legal and organizational standards. The RN may be required to perform on-call duties based on operational needs.

• Experience: Minimum of 0 years required; total experience of 1 year preferred.
