Job Summary:
Long Term Care (LTC) Nurse Practitioner opportunity in Beaver Dam, Wisconsin. This position involves providing care in a nursing home facility attached to a hospital, serving a majority private-residence population with diagnoses such as dementia, frail, elderly, and rehabilitation patients. The role emphasizes clinical duties such as charting, assessments, medication administration, and skill-based procedures including IVs, catheters, and ostomy care.

Job Details:

• Location: Beaver Dam, Wisconsin
• Schedule: 12-hour night shifts, with every other weekend and holiday coverage, minimum of 36 hours per week
• Start Date: 08/03/2026
• Required Qualifications: BLS (AHA); minimum of 1 year of experience; licensed to practice as a clinician, residing within 80 miles of the facility
• Experience or 'Must have' skills: Elderly patient care experience, clinical skills such as IVs, catheters, ostomy care, assessments, and charting
• Unit Notes: Department: LTC. The unit has an interdisciplinary team including therapy, social workers, activities, dietary, and housekeeping staff. It is equipped with pharmacy on-site, medication dispensing system, and RT on-site 24/7.
• Duties: Charting, patient assessments, charge duties, medication administration, clinical skills (IVs, caths, ostomy), and team collaboration.
• Additional information: Must note if clinician has previously worked for Marshfield; if yes, must have an end date at least 6 months prior to submission. All clinicians are required to clock in and out at the facility.
• Float: Float rotation may be required.
• Dress Code: Any solid color, clean, and professional attire.
