Job Summary:
Home Healthcare Registered Nurse located in Youngstown, Ohio. This position offers an opportunity to provide in-home patient care within a designated service area, focusing on geriatric populations. The role involves participating in on-call rotations, completing documentation promptly, and working Monday through Friday, 8:00 AM to 5:00 PM, with a minimum of 40 hours per week.

Job Details:

Schedule: Monday to Friday; 8:00 AM to 5:00 PM, minimum of 40 hours per week
Required Qualifications: Minimum of 1 year of relevant experience, OASIS experience required for staff completing Start of Care (SOC) in home health, proficiency in HCHB (Homecare Homebase) required, active drivers license with personal transportation, recent experience with patient ratios and charting systems, BLS certification
Certifications: BLS required
Special Requirements: Active drivers license, timely documentation and communication, flexibility in territory, scrubs dress code (any color), no cell phone, iPad provided, mileage reimbursed at IRS standard rate
On-call Requirement: Yes, may be required, with participation in a triage team for urgent visits, on-call rotation approximately 3 times per month
Territory: Up to a 60-mile radius from the hub/office, local candidates accepted
Additional Information: Scheduled orientation on Monday, travel pairs accepted but each must have own transportation, modules are billable and completed during orientation, no first-time travelers accepted, all travelers must have active and in-hand license prior to start, RTO requests must be submitted at application
