Job Summary

Home Health Nurse (RN) East Portland, Oregon. This position involves providing skilled nursing care to adult and geriatric patients within a home health setting. The role requires adherence to clinical guidelines and ensuring high-quality patient care through assessments, treatments, and patient education. The assignment starts immediately and offers day shifts with a standard workweek.

Job Details

• Schedule: 8-hour days, minimum of 40 hours per week
• Required Qualifications: One year of Skilled Home Health Experience, Oregon or Washington licensure, BCLS certification, COVID-19 vaccination (primary series and one booster), Oasis experience; local candidates considered within a 50-75 mile radius; pending license acceptance.
• Preferred Qualifications: Experience with wound care, IV starts, and electronic charting; experience with interpretation services; familiarity with home health procedures.
• Float: Float rotation may be required within the state, but this role is primarily focused on East Portland and surrounding areas.
• Additional Information: Candidates must have their own transportation, as occasional travel within a radius of approximately 60 miles is expected to minimize drive time. The facility emphasizes clinical competence in adult and geriatric care, including wound management, medication administration, and disease management. The assignment will utilize electronic medical records including EPIC and Rover.
