Description

Job Summary:
Home Health Registered Nurse (RN) Vermont. This full-time position involves providing individualized, goal-oriented nursing care within a community-focused, not-for-profit organization dedicated to enhancing the well-being of residents. The role includes functioning as a Case Manager when leading the discipline, ensuring comprehensive care delivery based on physician orders.

Job Details:

Schedule: Days shift, 12 hours per shift, minimum of 36 hours per week
Required Qualifications: Valid Nursing license, Education in Nursing, location in Vermont, 1 year of relevant experience or more, ability to work full-time hours, and ability to care for a diverse patient population
Preferred Qualifications: Experience in home health services, Certification in Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) preferred
Additional Information: Provide goal-oriented nursing care utilizing the nursing process in accordance with physician orders. Serve as a Case Manager when appropriate. Ensure compassionate, patient-centered approach to home health care.
