Job Summary
Home Health Hospice Nurse in West Virginia This role involves providing compassionate hospice care to terminally ill patients in their homes or nursing facilities within a designated service area. The position requires a dedicated professional to work between two registered nurses, ensuring quality end-of-life care aligned with established protocols.

Job Details

• Shift: Days, 8-hour shifts, Monday through Friday, minimum of 40 hours per week.
• Required Qualifications: WV license, Basic Life Support (BLS) certification through American Heart Association (AHA), minimum of 2 years of clinical experience in hospice or home health care.
• Preferred Qualifications: Experience with electronic health records, particularly EPIC, and familiarity with care for terminally ill patients with diagnoses including cancer, heart disease, lung disease, or dementia.
• Additional Information: The role covers seven counties including Harrison, Barbour, Upshur, Lewis, Taylor, Doddridge, and Wetzel, primarily serving patients in their homes or nursing facilities. Orientation will be provided for EPIC documentation and clinical procedures. The position is based at a facility in Elizabeth, West Virginia.
