Job Details:

• Provides high quality professional nursing services in a variety of settings.
• Responsible for walk-in and telephone triage.
• Coordinates patient care through collaboration with the patient, family, and healthcare team.
• Provides patient care by assessing, planning, implementing, evaluating, and documenting individual patient needs, actions taken, and patient responses.
• Responsible for case management based on primary care physician panels, in collaboration with network providers, including nurse visits and telephone management.
• Delegates or assigns aspects of patient care to healthcare team members based on their validated competency.
• Performs documentation within scope of practice.
• Performs other functions as required.
• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
• Required Qualifications: Registered Nurse (RN) license in the state, current Basic Life Support (BLS) certification, ADN or ASN from an accredited nursing program, and at least 1 year of nursing experience.
