Job Summary:
Hospice Registered Nurse (RN) position in a home hospice setting, covering Morris and Sussex Counties. This role involves performing patient assessments, developing and implementing care plans, ensuring appropriate utilization of services, documenting clinical activities, and participating in interdisciplinary team meetings. The schedule is night shifts, with weekend requirements every three weeks and holiday coverage. The position is ideal for experienced hospice nurses dedicated to providing quality patient care and case management.

Job Details:

• Schedule: 8-hour night shifts from 4:00 PM to 8:00 AM, three shifts per week, with a minimum of 40 hours per week.
• Required Qualifications: Valid New Jersey RN license, associate's degree in nursing or diploma, current BLS certification, at least 2 years of experience as an RN in a home hospice environment, experience with electronic medical records, and excellent critical thinking and communication skills. Valid New Jersey drivers license is also required.
• Preferred Qualifications: More than 2 years of RN experience, specifically within a hospice agency or organization.
• Additional Information: The role includes conducting patient assessments, developing care plans, aligning with hospice regulations and insurance reimbursement guidelines, supervising home health aides, participating in performance improvement activities, providing mentorship to new employees, and supporting agency growth through orientation and cross-training. The position requires adherence to infection control measures, safe medication management, and involvement in interdisciplinary team meetings every two weeks.
