Job Summary:
Hospice Registered Nurse (RN) Location unspecified. This hospice care position involves comprehensive patient assessments, care planning, medication reconciliation, and interdisciplinary communication within a home hospice environment. The role emphasizes ensuring appropriate utilization of services, patient and family education, and adherence to hospice regulations and insurance reimbursement guidelines.

Job Details:

• Schedule: Day shift (7:30 AM 4:00 PM), every other weekend, with schedule adjustments such as paid days off during the week when working weekends.
• Required Qualifications: - Current New Jersey Registered Nurse (RN) license; - Associate's (ASN) degree or diploma with RN license in New Jersey; - Valid New Jersey drivers license; - Must have two (2) years of experience as an RN in a home hospice environment; - Experience using an Electronic Medical Record (EMR); - Excellent critical thinking and communication skills; - Required certifications include BLS. - No minimum years of experience required.
• Preferred Qualifications: - Three or more years of RN experience, specifically within a hospice agency or organization.
• Additional Information: - Responsibilities include performing patient assessments, developing and revising care plans, ensuring documentation accuracy, maintaining infection control measures, participating in performance improvement activities, and providing mentorship to new staff. - The position requires managing medication reconciliation, patient education, and communication with the interdisciplinary team. - The role involves active participation in staff meetings and quality improvement initiatives. - The assignment duration is from 09/14/2026 to 12/13/2026, with scheduled work hours and responsibilities aligned to hospice care standards.
