Job Summary:
Home Health Hospice Nurse in a critical care setting, providing specialized end-of-life care to patients in a home health environment. This role involves delivering compassionate and comprehensive nursing services, ensuring patient comfort and adherence to care plans. The position is scheduled for evening shifts, with a focus on patient-centered care coordination and symptom management.
Job Details:
- Schedule: Evenings, 8 hours per shift, minimum of 40 hours per week.
- Required Qualifications: Valid nursing license, current Basic Life Support (BLS) certification, and at least 1 year of nursing experience in home health or hospice care. Education requirements include a nursing degree from an accredited institution. Location requirements specify candidates must be authorized to work in the designated region.
- Preferred Qualifications: Certification in hospice or palliative care, experience with electronic health records, and familiarity with end-of-life patient advocacy.
- Additional Information: The position involves providing end-of-life care in patients' homes, coordinating with interdisciplinary teams, and adhering to established protocols for hospice nursing. The assignment begins on 08/31/2026 and concludes on 11/29/2026.
