Job Summary:
Wound Care Nurse Conducts health assessment interviews focused on wound, ostomy, and continence related care. Performs focused physical assessments, develops individualized plans of care based on diagnoses, and recommends appropriate treatments and medications. The role involves collaboration with the healthcare team, patient education, staff training, and participation in quality improvement activities in a non-acute nursing setting.

Job Details:

• Schedule: Days shift; 7.5 hours per day; minimum of 37.5 hours per week; rotating weekends and holidays required
• Required Qualifications: Associates Degree (ASN, ADN) from an accredited nursing program; New York State RN License; primary source verification of license and registration from NYS Department of Education; AHA Basic Life Support (BLS); current certification in one of the following: Certified Wound Ostomy Continence Nurse (CWOCN) issued by the Wound, Ostomy and Continence Nursing Certification Board, Certified Wound Ostomy Nurse (CWON) issued by the Wound, Ostomy and Continence Nursing Certification Board, Certified Wound Care Nurse (CWCN) issued by the Wound, Ostomy and Continence Nursing Certification Board, Certified Wound Specialist (CWS) issued by the American Board of Wound Management, or Wound Care Certified Certification (WCC) issued by the National Alliance of Wound Care and Ostomy.

• Additional Information: The role includes assessing and documenting patient responses to wound, ostomy, and continence care, evaluating quality of care delivered by clinical staff, developing and providing staff and patient education, managing rented specialized equipment, recommends best practices, and participates in quality improvement initiatives related to wound and continence care.
